For many stakeholders, the experience of health care may feel isolated and fragmented. Currently, care management is limited to the windows of time that patients and providers have during in-person appointments to talk about quality of life, lifestyle habits, monthly device readings and adjust treatment . This is because providers only have access to a fraction of the valuable information about patients' health outside the four walls of the hospital.
However, the use of digital health devices and applications, such as medical devices and portable devices in the home, can extend care beyond these walls and provide a better understanding of the health of patients. These devices and applications generate valuable patient data that, when incorporated into the clinical workflow and into a continuous program of care, leads to more efficient and effective treatment that can reduce costs and improve outcomes.
In this webinar, three stakeholders – including a patient with type 2 diabetes, a provider who designed and managed remote care programs, and a health technician specializing in workflows and data analysis – will share their perspectives on the value of Patient-generated health data (PGHD) and digital health devices offer remote care programs.
Participants will also get practical guidance on:
– Best practices to implement and scale a remote care program with digital tools
-How it is better to involve and train patients with remote care programs based on data
Join the online seminar to hear a patient talk about their first-hand experience participating in a remote care program, and the perspectives of a technician and provider on the implementation and operation of those programs.
